Location

The seminar will be held in beautiful Cape Cod, at the Four Points by Sheraton Hyannis Resort, (508) 775-7775, in Hyannis Massachusetts.

 
Hotel Accommodations:
A limited block of rooms has been reserved at convention rates ($179/single/double) at the site hotel. These rooms will be assigned on a first request basis. Due to the large influx of people to Cape Cod in July, hotel space will be at a premium. To make your reservation, please call 1-800-598-4559 and ask for the SEAK/Workers' Compensation Seminar. NOTE: The seminar was sold out the last several years. Last year we overflowed into two additional hotels. Early registration is highly recommended.

Tuition/Discounts:
Tuition for the Twenty-fourth Annual Nat'l. Workers' Compensation and Occupational Medicine Conference is $695.00. This includes handout materials to be distributed at the door, coffee breaks, reception, three continental breakfasts, and one seminar luncheon. A 20% discount is available for two or more individuals registering together prior to May 15, 2004.

Upon receipt of your registration form and payment we will mail you a confirmation letter and driving directions.
 
Continuing Education Credit:
Continuing education credit is available for each program. Please see pages 3, 17, 18, 20, 22, 25 and 26 for more information.
 
Cancellations:
Cancellations received in writing before July1, 2004 will receive a full refund. Persons canceling after July 1, 2004, will not receive a refund, but will be provided with the course materials.
